SURGICAL INDICATION FOR DISSECTING ANEURYSMS OF THE AORTA

Out of 59 cases of dissecting aneurysm of the aorta, 38 cases have undergone surgical treatment in our hospital. According to the DeBakey's classification, there were 7 cases of Type I with 3 operative deaths, 8 cases of Type II with 1 operative death, and 23 cases of Type III with 3 operative deaths. Thus, entire operative mortality rate was 18%. All of the operation were performed at the subacute or chronic stage that means more than four weeks after the onset of acute aortic dissection. Cystic medial necrosis were the most frequent findings of the pathological examination, but the atherosclerotic changes were also observed frequently in Type III dissecting aneurysm. Surgical intervention was consisted of an end-to-end anastomosis of stumps of the aorta supported by a prosthetic media in patients of the Type I and II dissecting aneurysms. These simple and easy technique of aortic reconstruction allowed good follow-up results.
