TREATMENT OF DISSECTING ANEURYSM OF THE AORTA

2nd Department of Surgery, Niigata University School of Medicine

M. Terashima, S. Eguchi

Thirty-six patients with aortic dissection were seen during past seventeen years, 12 of whom had type 1 dissection and 22 type 111 by DeBakey' criteria, but in two cases, initial intimal tear was located in the upper abdominal aorta. Of 36 patients, 12 were seen in acute stage (within two weeks of onset), and 24 in chronic stage. Eight of the patients with type 1 dissection (acute stage: 4 cases, chronic: 4) and fourteen of the patients with type 111 dissection (acute; 5, chronic; 9) were treated surgically.
Early surgical management is indicated in patients with ascending aortic dissection, however, patients with descending aortic dissection can be treated medically initially followed by elective operation.
