TWO CASE REPORTS OF MULTIPLE SUBMUCOSAL CYSTS OF THE STOMACH ASSOCIATED WITH DOUBLE GASTRIC CANCERS
Recently, we encountered two cases having multiple submucosal cysts of the stomach associated with double gastric cancers. On these two cases, we discuss etiology and malignant potential of this disease. The patients were 71 year-old and 70 year-old male. They were found with gastric cancers and multiple polyps by upper G-I series and gastric endoscope. Gastrectomy revealed both multiple polyps to be multiple cysts of the submucosal layer with the co-existence of double gastric cancers. (The first case had Borrmann III advanced cancer and IIc+ Ila early cancer and the second case had I and Ila early cancers.) On cut section, each hypertrophic submucosal layer had many cysts. Microscopically, these cysts lied scattered in the submucosal layers and each cyst was surrounded by smooth muscle bundles. The walls of these cysts were composed of a single cuboidal or columnar epithelium layer. From above, we diagnosed them as the enterogenous cyst. As we found chief cells and parietal cells in these enterogenous cysts, we suspect that its etiology will be duplication. By the way, this disease often co-exists with gastric cancer and there are many reports on malignant change of it. Also, our two cases had double gastric cancers. The submucosal cysts of our two cases penetrated the muscularis mucosae and extended to the mucosa. Since we found atypical cells in these cyst walls, we guess this lesion to be a precancerous stage.
