Serial clinical observations and hemodynamic measurements with the use of Swan-Genz catheter were made on 22 patients with Sepsis.
Patients were divided into two groups according to their cardiac index (CI) determination : 1) hyperdynamic state, CI greater than 3.1 L/min/m2, and 2) hypodynamic state, CI less then 3.0 L/min/m2.
Patients who showed hyperdynamic hemodynamics recovered without any complication.
The prognosis was very poor in patients who were in hypodynamic state. Pulmonary insuficiency, renal failure and massive upper gastrointestinal bleeding were commonly observed in these patients.
Pharmacological dosis of steroides were administrated in 9 patients. In 6 cases of hypodynamic state the cardiac index was increased with decreaseing of total peripheral resistance. Of the 3 cases of high flow state, 2 cases showed increasing of cardiac index and decreasing of total peripheral resistance while one showed decreasing of cardiac index and increasing of total peripheral resistance.
