APPLEBY'S OPERATION FOR ADVANCED CARCINOMA OF THE STOMACH

During 9 years from 1969 to 1978, 245 patients of gastric carcinoma were operated on by total or proximal gastrectomy with removal of the entire celiac axis and its attendant lymphnodes en bloc which was advocated by L. H. Appleby in 1953.
Of the 245 patients, 218 were advanced carcinoma excluding superficial cancer whose invasion was limited to the mucosa and the submucosa. Operative mortality refers to the death within thirty days after surgery was 2.4%. Cumulative five-year-survival rate was 42% and nine-year-survival rate was 33%.
These results were extremely excellent comparing with the results of other gastrectomized patients who were operated on using several procedures except Appleby's method.
I am convinced that Appleby's operation is a definite contribution to the surgical therapy for advanced carcinoma of the stomach.
