PAST, PRESENT AND FUTURE IN CARDIOVASCULAR SURGERY
-CONGENITAL HEART DISEASE-
Current trend in surgery for congenital heart disease

There are two major trends in cardiac surgery for congenital heart disease at present. These are as follows.
1) New operative procedure with less residua and less sequelae
VSD: We have developed trans-pulmonary arterial method to close supra-cristal VSD.
PDA: Two-third of our patients underwent plug closure without thoracotomy in these 7 years.
TGA: Anatomical correction was performed in three patients and one survived and well.
TF: We have developed corrective operation without or with minimum right ventriculotomy and complete repair of the pulmonary valve. This principle was applied in 18 patients and 17 survived.
2) New palliative operation which improve anatomical situation of the heart and great vessels
TF: For the patients with small pulmonary arterial tree, shunt procedure were indicated not only for symptomatic palliation but also to enlarge the pulmonary artery. Thus, some patients, who considered to be inoperable before shunt procedure, underwent second stage corrective procedure.
SV: For the patient with small ventricular volume, who are otherwise considered to be a candidate for septation, shunt operation are now indicated to increase the ventricular volume.
