SLEEVE LOBECTOMY FOR LUNG CANCER AND THE RESULTS

Sleeve lobectomy resection for lung cancer was accomplished in 13 patients. The cell type of these patients were 10 cases of squamous cell carcinoma and every one case of adenoacanthoma, adenocarcinoma and carcinoid tumor.
In deciding the eligibility of bronchoplastic surgery, bronchial arteriography is an indispensable examination, because it offers the most valuable information about the extent of the tumor infiltration into the bronchial wall and the peribronchial structure which might not be detected by bronchoscopical observation. Instead of preoperative irradiation, anti-cancer agents were infused into the bronchial artery seven to ten days before the operation.
One case developed bronchial stricture due to granulation five months after the operation. Successful re-anastomosis was done between intermediate bronchus and the lower end of the trachea following resection of the stricture site.
As a whole, curative resection were done on 5 cases, relative curative resection on 6 cases and the other two were non-curative resection. All the patients have been on the protocol of immunochemotherapy selected to their cell type. For six cases with pN2, postoperative radiation therapy was employed. Two cases died at 19 months and 6 months after surgery. Although the longest one is four years observation, 11 cases are alive and well.
