HEPATIC PORTOJEJUNOSTOMY

Hepatobiliary diseases which have a stricture or obstruction of the bile duct at a level higher than the porta hepatis (such as intrahepatic gallstone or carcinoma of the bile duct) have been said to be almost unaccessible.
We have recently established an easy and safe surgical technique-hepatic portojejunostomy which can be applied to those conditions. The merits of our technique are summerized as 1) more drastic removal of intrahepatic gallstones is possible by this procedure than by traditional extended choledochotomy, 2) reconstruction of biliary passage is quite safely done between hepatic parenchyma and the jejunum and 3) a transhepatic tube used in the procedure can be utilized for repeated irrigation for removal of residual stones or an endoscope can be introduced through the same route after the tube is taken out.
We have performed this surgical method in 13 cases of intrahepatic gallstone, 3 cases of bile duct cancer and 3 cases of choledochal cyst and obtained a satisfactory result in all of those cases.
