PROBLEMS IN THE TREATMENT OF ACUTE THROMBOSIS DUE TO ARTERIOSCLEROSIS

Forty one patients with arterial thrombosis were analysed. Among of 28 cases treated by the surgical reconstruction, 17 cases (60. 7%) represented successful results, however, diseased extremities were amputated in 7 cases (25%). Medical treatment were carried out in 13 cases, in which extremities survived in 8 cases (61.5%) and were amputated in 5 cases (38.5%). Thus, surgical treatments were essentially much better than medical therapy, however, the mortality rate was higher in the former than in the latter. The main cause of death of 9 cases (22%) in our series was myonephrotic-metabolic syndrome (4 cases).
Biochemical and enzymological changes were daily examined in 20 patients. Rhabdomyolysis, nephrosis and metabolic acidosis were observed in 6 cases (30%) with severe extensive ischemia. On those changes, there is no practical difference between de-and revascularization phases.
Experiments using 50 dogs demonstrated various pathophysiological changes in myonephroticmetabolic syndrome.
A definite principle of treatment for arterial thrombosis were discussed based on those clinical and experimental studies.
