ENDOCRINE RESPONSE TO SUBTOTAL THYROIDECTOMY IN PATIENTS WITH GRAVES' DISEASE

To clarify the endocrine response to subtotal-thyroidectomy in patients with Graves'disease, the changes in the serum levels of anterior pituitary, thyroid, adrenal hormones and plasma renin activity were investigated before, during and after the subtotal-thyroidectomy in 46 patients with Graves' disease.
In the anterior pituitary hormones and cortisol, no significant differences were observed compared with the endocrine responses to general surgery. Among the thyroid hormones, T3 was decreased markedly after the operation and reached to a half of preoperative value on the 1st postoperativeday. After then, T3 value remained in hypothyroid level till the 7th postoperativeday. Both adrenalin and noradrenalin concentration increased transiently during surgery, but the level of noradrenalin was again increased significantly on the 3rd postoperativeday. Plasma renin activity was also increased in parallel with noradrenalin.
These results suggest that peculiar rise in noradrenalin or in plasma renin activity on the 3rd postoperativeday due to the transient hypothyroidism after subtotal thyroidectomy.
