THE RESULTS OF TRANSSPHENOIDAL ADENOMECTOMY IN HYPERSECRETING PITUITARY ADENOMAS

During the past four years we have operated on 93 hypersecreting pituitary adenomas by transsphenoidal approach.
Of 23 patients with Cushing's disease pituitary adenomas were verified in all exept one. Remarkable clinical improvement with decreased plasma ACTH and cortisol is obtained in 20 cases. Substitution therapy of glucocorticoid was necessary for several months. According to cessation of hypercortisolemia the anterior pituitary functions have improved in most cases.
Fourty-six patients with acromegaly were underwent surgery. Althoug most of them had macroadenoma, postoperative serum GH levels dropped below 10 ng/ml in 25 cases. Abnormal GH response to TRH or LHRH became normal in 10 cases.
In 10 out of 24 patients with hyperprolactinemia postoperative prolactin levels became normal by surgery alone. 16 cases resumed regular menstruation and 6 became pregnant with or without CB-154 administration after surgery.
In conclusion, transsphenoidal adenomectomy is effective in treatment of hypersecreting pituitary adenomas for clinical and endocrinological improvement.
