RECENT PROBLEMS ON PANCREATODUODENAL CANCER CLINICOPATHOLOGICAL STUDY ON RESECTABLE CASES

Of the total 204 patients with pancreatoduodenal cancer since 1960, 116 were carcinoma of the head of the pancreas, 56 were carcinoma of the distal common bile duct and 30 were carcinoma of the papilla of Vater. These resectable cases were 26, 34 and 20, respectively. The resectable rate of pancreatoduodenal cancer was 39.2%. Hospital deaths within one month after the operation were 6 patients (7.5%).
According to the comparisons of the histological study and prognosis in resectable cases, the following results were obtained.
1) The 5 year survival rate was 14.3% with carcinoma of the head of the pancreas, 17.4% with carcinoma of the distal common bile duct and 38.5% with carcinoma of the papilla of Vater.
2) The survival period was relatively short in poorly differentiated adenocarcinoma.
3) The survival period was short in carcinoma of the head of the pancreas and carcinoma of the distal common bile duct with INF-γ.
4) The survival period was significantly short in carcinoma of the head of the pancreas with venous invasion and carcinoma of the distal common bile duct with duodenal invasion and pancreatic invasion.
5) The survival period was short in carcinoma of the head of the pancreas and carcinoma of the dstal commoin bile duct with Si.
6) The survival period of carcinoma with lymph node involvement was shorter than carcinoma with no lymph node involvement, specially in carcinoma of the distal common bile duct the survival period was significantly short.
7) The cause of death in carcinoma with lymph node involvement was mainly recurrent cancer within two years after surgery.
