PERINEAL RECURRENCE AFTER RADICAL OPERATION FOR CARCINOMA OF THE EXTRAPERITONEAL RECTUM

Relationship between the distance from the external edge of tumor to the surgical margin of resection(ew) and the local perineal recurrence was studied in 216 patients with extra peritoneal rectal carcinomaoperated curatively.
There was no recurrence in patients with carcinoma localized within the mucosa or submucosa. In cases of carcinoma which invaded into the proper muscle layer, there was no significant difference among no recurrent cases, perineal recurrent cases and distant metastatic cases. In cases of carcinoma which reached the serosa, the longer the length of "ew" was, the smaller the incidence of the local recurrence was. The mean value of "ew" was 5.98 mm in no recurrent cases, 2.28 mm in perineal recurrent cases and 6.48 mm in distant metastatic cases. There was a significant correlation between the "ew" length and the perineal recurrence in cases of carcinoma which reached the serosa.
Other factors such as histological type, lymph node involvements, vascular invasion and INF have influence on the relationship between the "ew" and the perineal recurrence.
