A STUDY OF MITRAL STENOSIS WITH BALL VALVE LIKE THORMBUS
―DEFFERENTAIL DIAGNOSIS BETWEEN BALL VALVE LIKE THROMBUS AND LEFT ATRIAL MYXOMA BY ECHOCARDIOGRAM―

We studied a case of mitral stenosis with ball valve like thrombus, discussd especially the echocardiographic findings of the case trying to differenciate it from our six cases of the left atrial myxoma.
A common echocardiographic finding of both diseases showed a movavle tumor echo in the left atrium, and the tumor echo were getting close to the anterior mitral leaflet during diastolic dhase.
Differential findings were as follows.
1) In the case of mitral stenosis with ball valve like thrombus, the diastolic slope (E-F slope) of the anterior mitral leaflet was reduced, echo was strong and C-E amplitude was decreased.
2) In the cases of left artial myxoma, strength of leaflet echo were within normal range and showed spiks at early diastolic phase and initial diatolic slope were within almost normal range.
3) Echo-free interval were shorter in myxoma than in ball valve like thrombus, and were longer when myxoma was small.
4) Two cases of myxoma showed paticular echos. One of them showed long echo-free interval in spite of a large myxoma, and the other's anterior leaflet echo was stong and showed plateau pattern.
