Operative results, techniques and supporting devices for aortic clamping were discussed in 42 patients with various types of the anemysms.
Ten patients died soon after operations with operative mortality of 21.4%. Another 3 died in the late period (7.4%).
In dissecting aneurysms, anastomosis of the prosthesis was inforced with teflon felet placed in and/ or arround the pseudolumen. This inforcement of the anastomosis was effective to minimize the postoperative bleeding. However, in the far advanced cases, still bleeding of the anastomosis was a troublesome problem. On the contrary, in relatively early stage of this disease after attack of dissection, anastomosis was easy without bleeding.
On the supporting devices to maintain the blood supply to the lower half of the body while clamping the first choice and if this procedure is difficult to perform due to adhesion, V-A bypass was adopted.
