CORONARY ARTERY SURGERY WITH A PARTICULAR REFERENCE TO SYPHILITIC MESOAORTITIS

Aortocoronary artery bypass procedure has become a very popular technique for ischemic heart disease and also been able to apply to various pathologic conditions of the coronary artery.
In patients with syphilitic mesoaortitis, it is reported that 27 per cent was accompanied by stenotic or obstructive lesion of the coronary arterial ostium and in some of these, aortic insufficiency or aneurysmal formation of the aorta or both were combined simultanously. A patient with aortic insufficiency and right cornary ostial stenosis of syphilitic origin was described in this report and particular problems concerning these lesions were discussed.
In this connection, the simultaneous surgical treatment of aortic replacement and cornary bypass has to be mentioned, and this type of surgery resulted in high mortality initially. To improve surgical outcome, it has been adviced that maximal myocardial protection during valve replacement should be maintained.
