HISTOLOGICAL TYPES OF LUNG CANCER AND SURGICAL TREATMENT
-INDICATION OF OPERATION FOR SMALL CELL CARCINOMA-
Small cell carcinoma has distinct clinical and pathological features different from other types of lung cancer.
General principle of the treatment for small cell carcinoma is still contraversial because of very poor prognosis after surgery and of good response to chemotherapy and/or radiotherapy.
In order to evaluate the efficacy of surgical treatment for small cell carcinoma as compared with chemotherapy and/or radiotherapy, 37 operated cases were analysed in regard to various prognostic factors. Those cases with oat cell type, showing involvement of mediastinal lymph nodes and undergoing non-curative resection were mostly dead within one year. Those cases undergoing pneumonectomy for hilar type tumors did not survive two years. Long-term survivors were found only in cases with peripheral nodules, non-oat cell type, no involvement of lymph node and curative resection.
Histologically, as to the nuclear pattern, large nuclei, marked irregularity of the shapes, irregular distribution of chromatins and prominent nucleoli were frequently observed in the group of favorite prognosis in contrast to small round nuclei, fine dense chromatins and indistinct nucleoli in the group of poor prognosis. On the other hand, the cases with the latter findings responded much better to chemotherapy. These cytological findings may be contributory for choice of treatment for small cell carcinoma.
