TREATMENT FOR BUERGER'S DISEASE-CONSERVATIVE MANAGEMENTS

Ⅱ Department of Surgery, Hamamatsu University School of Medicine

It is important for salvaging the limb in Buerger's disease to apply freely not only surgical procedures but also suitable medical managements.
As recent advance in the treatment for severe ischemic lesions of the extremities, 3 drug therapies were discussed.
1) Prostaglandin E1 , which was continuously infused into the arteries of diseased limbs (0.15 ng/ kg/min.), was significantly more effective as compared with two other treatments.
2) It was clarified in the experimental studies that excellent clinical results obtained from the local administration therapy of corticosteroid for severe ischemic lesions of the extremities may be based upon the cell lysosome stabilizing effect of corticosteroid.
3) The effect of peroral therapy of Cytochrome-C for ischemic pain and ulcer of the extremities was statistically not different from the results obtained by Kallikrein therapy which has been generally accepted as a effective therapy for such disease.
In conclusions, those therapies may be of value for the patients who have intractable ischemic lesions of the extremities due to Buerger's disease.
