HEPATECTOMY FOR PRIMARY LIVER CANCER WITH LIVER CIRRHOSIS

We have performed 43 hepatectomies for the patients with hepatoma from 1964 to 1977. These cases consisted of 36 flank type cancers and seven occult type cancers. The latter could be detected or diagnosed by the increase of serum alpha-fetoprotein. Out of 43 cases 29 had liver cirrhosis. Five year survival rate of the cases with liver cirrhosis was 6%, whereas those without cirrhosis was 34%. Among the patients with cirrhosis postoperative hepatic insufficiency was frequently observed and this made the postoperative mortality rate higher, 24% of this group.
To conquer these postoperative complications, we have introduced insulin supplimentary therapy and it was very effective for some patients with hepatoma and cirrhosis and it correlated with prognosis.
