The congenital intercostal arteriovenous fistula is very rare, we could find out only two cases in the literature published untill today. A 4-year-old boy who had the continuous murmur in the right scapular area, was diagnosed as congenital right 6th intercostal A-V fistula by selective arteriography. The A-V communication consisted of single channel fistula between the 6th intercostal artery and vein, and there was a large shunt to the superior vena cava through the azygos vein. The operation was perfomed by division of 6th intercostal artery in the origin. Even 14 years after operation the patient is doing well without recurrence of A-V fistula.
