STUDIES ON POSTOPERATIVE BLOOD PRESSURE RESPONSE IN RENOVASCULAR HYPERTENSION RELATING WITH PATHOPHYSIOLOGICAL BEHAVIOR OF THE AFFECTED KIDNEY

During past 16 years, 81 patients were surgically treated for renovascular-hypertension. Open biopsy was performed in 42 cases at the time of surgery. Histopathological findings of the renal tissue distal to the arterial stenosis, either preoperative or postoperative value of the plasma renin activity (PRA) and preoperative and postoperative findings of the intravenous pyelography (IVP) were retrospectively correlated with postoperative blood pressure response.
1. Ischemic changes such as mural thickening of arteriole, atrophy of tubulus, interstitial fibrosis and cell infiltration, hyalinization of glomerulus and hyperplasia of juxtaglomerular apparatus were observed in all 42 histological specimens of the renal tissue distal to the arterial stenosis, and 7 cases, in which severe ischemic changes were observed, did not attain the patency of reconstructed renial artery.
2. The PRA value shifted in a specific pattern after successful sugery, i.e., transient high value occurred immediately after surgery, and returned to normal value within a week.
3. In regard to the PRA value which was measured at the time of surgery, the ratio of renal vein PRA value in the kidney with stenosed artery as compared to that in the opposite one exceeded 1.5 in successfully operated cases, and furthermore, the renal vein PRA value of the kidney whose renal artery was stenosed exceeded 50ng/ml/hr.
4. Concerning the length of the longitudinal axis of the kidney, the axis of the affected kidney was apparently shorter than that of the opposite in preoperative period, but after the successfully performed surgery it increased in length, and we could not find any difference between them with mean follow up period of 7 years.
