THE CHANGES IN FREE WATER CLEARANCE RESPONSE TO FUROSEMIDE ADMINISTRATION IN SURGICAL PATIENTS
The changes in free water clearance (C
H2O) before and after intravenous administration of furosemide (FM) werestudied in 110 surgical patients.
The results obtained are as follows;
1) The patterns of C
H2O change responsing to FM were classified into 4 groups. The C
H2O value markedly elevated to a positive in group A and decreased even lower in gruop B 30 minutes after injection of FM and then almost recovered to the previous value in both groups 120 minutes after injection. In group C, the C
H2O value elevated gradually after injection but remained below zero. In group O, the C
H2O kept its value close to zero.
2) Pattern A was observed 21 times (35%) in preoperative patients and 13 times (15%) in postoperative patients. Those patients were thought to have a normal response to FM and to be in a well-hydrated condition.
Pattern B was observed 23 times (38%) in preoperative patients and 53 times (60%) in postoperative patients. Those patients were also thought to have a normal response to FM but to be in a poorly hydrated condition .
Pattern C was observed 10 times (17%) in preoperative patients and 9 times (10%) in postoperative patients. This pattern was observed in patients with diseases of the hepato-biliary tract such as obstructive jaundice or liver cirrhosis.
Pattern O was observed 6 times (10%) in preoperative patients and 13 times (15%) in postoperative patients. Those patients showed no response to FM and then subsequently developed acute renal failure.
