CALORIC AND AMINO ACID REQUIREMENTS IN TOTAL PARENTERAL NUTRITION

To assess the adequate calorie-amino acid requirements for total parenteral nutrition, extensive nitrogen balance studies were performed. Indirect calorimetry was also carried out to evaluate the adequacy of calorie level s thus obtained. Results : (1) In the patients without surgical stress, resting metabolic rate (RMR) ranged between 28 to 33 cal/kg, being less than 20% above the predicted basal metabolic rate (BMR). (2) In this group, regression analysis indicated that nitrogen balances can be achieved at a intake of 28 cal/kg and 1.0 g/kg of amino acid, or 32 cal/kg and 0.8 g/kg of amino acid. (3) With the caloric intake fixed at 40 cal/kg, the nitrogen balance to zero was achieved by supplying 1.5 g/kg of amino acid after major abdominal surgery. (4) In more critical septic patients over 50 cal/kg and 2 g/kg of amino acid were needed to attain positive nitrogen balance.
