CLINICAL AND EXPERIMENTAL STUDY ON THE GENESIS OF ANEURYSMAL FORMATION FOLLOWING TRANSPLANTATION OF VASCULAR PROSTHESIS

Department of Surgery, Tohoku University School of Medicine

Hisao Sasaki, M.D.

In order to study the causative factors of aneurysm formation after vascular anastomosis, a review of the clinical records of patients with such aneurysms, pathological study and animal experiment were done.
1h 14 aneurysms (in 11 cases) after transplantation of vascular prostheses have beensencountered at our surgical department in the past 15 years. The review of these patients suggests that arteriosclerosis and mechanical stress play an important role in the development of an anastomotic aneurysm.
2h Histopathological study was done concentrating upon the following 3 points, ( 1 ) degree of organization of transplanted grafts ( 2 ) tissue reaction of the graft ( 3 ) structure of the host artery at the site of rupture. This examination suggests that poor organ zation, marked tissue reaction and frag lity of the artery are important en the development of an anastomotic aneurysm ;
3) In animal experiment, the tensile strength of vascular anastomotic line was measured with a self-made tensiometer ; This examination suggests that good organization of vascular prosthesis is sufficiently strong to hold the anastomosis.
