STUDIES ON THE HISTOPATHOLOGICAL AND ROENTGENOGRAPHIC FINDINGS OF THE RESECTED ESOPHAGUS WITH ESOPHAGEAL SUBMUCOGRAM

Esophageal submucography was one of the diagnostic examinations in the patients with esophageal cancer. The procedure was done using esophagofiberscope. Contrast medium was injected into the submucosal layer at the proximal side of the esophageal cancer. The submucogram was roentgenogram applied the esophageal submucography.
In our department, esophageal submucographies were performed on 23 patients with esophageal cancer and 19 cases were studies on the histopathological and roentgenographic findings of the resected specimens.
The followings acquired by these studies.
1) Type 1 . Submucograms showed spread of contrast medium stopped at the upper margin of tumor in 8 cases. Contrast medium was interrupted at all layers of the upper margin of tumor on the roentgenographic and histopathological findings in these cases.
2) Type 2 . Submucograms showed spread of contrast medium penetrated into the outside of the cancer tissue in 5 cases. Contrast medium diffused to the adventitial layer beyond the cancer tissue on the roentgenographic and histopathological findings in these cases.
3) Type 3. Submucograms showed spread of contrast medium was interrupted at the depth of the cancer tissue on the roentgenographic and histopathological findings in these cases.
4) The preoperative radiotherapy and chemotherapy were necessary to cases in Type 1 and Type 3, but not necessary to cases in Type 2 .
5) Esophageal submucography was useful for preoperative diagnosis of the adventitial invasion in esophageal cancer.
6) In several cases, the connective tissues were produced by the preoperative radiotherapy and chemotherapy at the outside of the tumor. In these cases, contrast medium was interrupted at the upper sides of tumor bacause of the connective tissues.
Conclusion
On roentgenographic and histopathological findings, esophageal submucography was available to decide possibility of the radical operation of the esophageal cancer.
