STUDIES ON THE ESOPHAGEAL SUBMUCOGRAPHY

Esophageal submucography was one of the diagnostic examinations in the patients with esophageal cancer. The procedure was performed with using esophagofiberscope. Contrast medium, Myodil 5 ml, was injected into the submucosal layer at the proximal side of the tumor of the esophagus throuth the esophagofiberscope. The submucogram was x-ray film applied for the esophageal submucography. This was a very useful examination in order to determine the possibility of radical operation of the esophageal cancer. The procedures used to obtain the submucogram was simple.
In our depertment, esophageal submucographies were performed on 16 patients with esophageal cancer. The submucograms revealed the margin of cancer tissur. Contrast medium was injected into the submucosal layer of the esophagus in patients, and spread into the normal muscular and adventitial layer, but never penetrated into the cancer tissue.
The submucograms are classified as follows :
Type 1 : The submucograms showed that spread of contrast medium was stopped at the cancer tissue. 7 patients belonged to this group. In these cases marked adventitial infiltration was revealed by operation.
Type 2 : The submucograms showed that spread of contrast medium was penetrated into the outside of the cancer tissue. 4 patients belonged to this group. In these cases slight adventitial infiltration was revealed by the operation.
Type 3 : The submucograms showed that spread of contrast medium was to the depth of canser tissue. 5 patients belonged to this group. In these cases only moderate adventitial infiltration was revealed by the operation.
The spread of contrast medium shown by the submucographies of esophageal cancer was in accord with and proportion to the cancer adventitial infiltration. This was recognized by the examinarions of the resected specimens.
