THE ENTITY OF BUERGER'S DISEASE
-PATHOLOGIC STUDIES-
The entity of Buerger's disease was investigated based on pathologic studies. Resected segments of occluded artery in 14 extremities of twelve cases with Buerger's disease and four amputated limbs were examined. Thrombosis and embolism were produced in femoral artery in dogs and histological changes of occluded artery were compared to clinical materials mentioned above.
The following results were obtained.
1) In acute stage, granulomatous inflammation including giant cell around thrombus was considered to be pathogenomonic feature to Buerger's disease.
2) In subacute stage, organization of thrombus rapidly developed and granulomatous inflammation turned to nonspecific one.
3) In chronic stage, organized thrombus was still cellular and rich of recanalization. Structure of the media was well preserved, though destruction of internal elastica was rather prominent.
4) No inflammatory changes developed in the arterial wall in experimental thrombosis and embolism.
5) Inflammatory mechanism was considered to be the most important one, although many factors such as abnormality of blood coagulability and chronic trauma contributed to development of Buerger's disease.
