STUDY ON INDICATION TO SURGICAL TREATMENT FOR GASTROPTOSIS, ESPECIALLY SEGMENTAL RESECTION OF BODY OF THE STOMACH

Surgical treatment for gastroptosis, especially plastic surgery of the stomach, requires strict indication, without which favourable results cannot be obtained.
Regarding to the indication, foundamental studies were done on 948 cases of gastrpotosis observed at this depratment, and pre-and post-operative clinical and physiological studies were undertaken on 223 cases which had various types of surgical treatments.
The following cases are indicated to surgical treatments : 1. Encephalogram as a test of the central and autonomic nerous system showed normal or depressed function of the inter-brain; 2. Sensisitivity to Insulin as a test of function of the pituitary and adrenal system showed normal or resistant type; and 3. Curves of the gastric movement showed being accelated with Vagostigmin.
Contraindications are; haveing tonic typs of inter-brain encephaogram, sensitive to Insulin the marked disturbance of sugar assimilation.
Study on secretion of gastric juice resulted no standard of surgical indication. Normoor hypo-acidity is a choice of the segmental resction, and hyper-acidity is indication of gastrectomy ; resection of the pyloric portion of the stomach with modified Billroth 1. type anastomosis.
Follow-up study revealed that favourable results were obtained in cases which had had severe gastric symptoms than the other subjective complaints. Gastropexy, gastrectomy and segmental resection of the stomach were compared. The best result was obtained on the segmental resection, which fits to the purpose and permits wide indication.
