STUDIES ON THE HAEMODYNAMICS IN THE LIVER

The bromsulfalein (BSP) dye infusion method combined with the intravenous catheter technique based on the Fick principle is not applicable for the estimation of the hepatic blood flow during operation. Because there is no significant difference in the BSP concentration between the peripheral and hepatic veins .
In the present paper, the disapperance rate of colloidal radio gold (Au 198) injected intravenously is observed to estimate the hepatic blood flow during operation.
1) As a preliminary experiment, both concentrations of Au 198 and BSP in the peripheral and hepatic veins have been estimated before or during operation. As for Au 198, its extraction and uptake by the liver have also been measured. Consequently the values obtained by the both methods above mentioned were compared to each other.
2) The hepatic blood flow of 50 patients suffering from upper abdominal diseases were measured . A remar kable correlation between H.B.F. and such things as circulating blood volume and results of the liver function test, particularly, the BSP test, Meulengracht test. albumin, γ-globulin and A/G ratio are recognized.
3) There is a parallel relationship among the H.B.F., liver temperature and the bile volume obtained from the common bile duct at every 15 minutes during operation. H.B.F. and the liver temperature are decreased with the depth of anaesthesia. The above decrease is more remarkable in the spinal anaesthesia than in the general anaesthesis. It is very interesting result that H.B.F. restores the normal prior to the liver temperature.
4) It appears that H.B.F. easily adapts against every stress.
