A CASE OF CYSTIC ADVENTITIAL DISEASE OF THE POPLITEAL ARTERY
Since Ejrup and Hiertonn had reported the first three cases of the cystic adventitial degeneration of the popliteal artery in 1954, there has been no case report in the world literature except only one case added by Hiertonn, Lindberg and Rob in 1956.
Recently, the authors found a case, a 32 year old male farmer, of trasient arterial occlusion of the right lower limb caused by this disease.
The authors analysed the circulatory condition of the involved limb physiopathologically, and detected the following characteristic findings :
1) The chief complaint was intermittent claudication.
2) Sudden onset of symptoms including cramps in the calf after the flexion of the knee joint over 90°.
3) Irregular course and circumscribed stricture of the popliteal artery were detected by femoral arteriography .
4) Amplitude of the pulse wave, the arterial inflow and the oscillometric readings of the involved foot diminished extremely after walking or flexion of the knee joint.
5) A ganglion-like cyst limited in the adventit ia was confirmed by the operation.
6) With extirpation of the cyst, the subjective complaint was relieved completely.
7) From above-described findings the authors named this clinical entity the cystic adventitial disease of the popliteal artery.
