STUDIES ON THE DIAGNOSIS OF CECUM MOBILE AND INDICATIONS FOR ILEOCECAL RESECTION
Roentgenographic and clinical studies on 45 cases of cecum mobile revealed the following results :
1) Viewed roentgenographically, patients with cecum mobile were classified into the following two groups: (1) The simple syndrome group consisting of those who show abnormal mobility of the cecum and lower portion of the acending colon, but are free from morbid changes in other portions of the gastrointestinal tract. (2)The partial syndrome group comprising those who combine abnormal mobility of the cecum and lower portion of the ascending colon with gastrocoloptosis.
2) There were no significant differences between the two groups of patients in observations made in the small intestine as well as in mobility, form and displacement of the cecum.
3) While many patients of the simple syndrome group were asociated with such clinical symptoms as pain in the right lower abdomen, constipation of medium degree and a sense of tension, many patients belonging to the partial syndrome group had such symptoms involving the body as a whole as a sensation of general fatigue, headach and loss of appetite, in addition to local symptoms.
4) The results of ileocecal resection in cases of cecum mobiIe revealed that the operation achieved a success in patients belonging to the simple syndrome group. It was shown that, if there was pain in the right lower abdomen, ileocecal resection was indicated.
5) It was shown that ileocecal resection was not indicated in patients belonging to the partial syndrome group, and that a sense of tension and constipation of high degree were contraindications for ileocecal resection.
