CLINICAL STUDIES ON AORTOGRAPHY FOR VASCULAR SURGERY
PART I CLINICAL STUDIES ON THE THORACIC AORTOGRAPHY

Clinical studies on angiography have been performed at Prof. Kimoto's Surgical Clinic in the Tokyo University Hospital since 1949. In the last 6 years between March 1952 and the end of February 1958, angiographies have been performed on 296 patients admitted to Prof. Kimoto's Surgical Clinic. They are 35 cases on the thoracic aorta, 64 cases on the abdominal aorta, 135 cases on the peripheral arteries and 62 venographies.
This report includes the technique of the thoracic aortography, the selection of contrast media used in the thoracic aortography, its diagnostic value and some considerations on the experienced 35 cases, and the following results have been obtained.
1) As to the technical problem of the thoracic aortography, the retrograde method inserting vascular catheter through the brachial or profunda femoral artery is the safest and sure method.
2) The catheter of 1 meter in length should be more than F 10 in diameter for retrograde angiography to obtain a satisfactory visualization as the aid of vascular surgery.
3) The brain complication caused by contrast media was most frequently (23.5%) seen by Pyraceton (Diodrast) and one of the patients died of the complication. No such complication was noted by Urografin. Consequently, Urografin is the safest contrast media for the brain complication.
4) Thoracic aortography reveals the relations of diseased site and area and its neighbouring organs and makes the anticipation of prognosis possible.
5) As the representative cases from 35 cases with the thoracic aortography, 5 cases of thoracic aneurysm and 2 cases of mediastinal tumor were presented and discussed here showing the necessity of the thoracic aortography.
