HISTOPATHOLOGICAL STUDY OF GASTRIC MUCOSA IN CHRONIC GASTRITIS
1) I have made a histological, especially histotopographic study (Morson's point system) of 52 cases of gastrectomized primary chronic gastritis, consisted of 22 bleeding cases and 30 nonbleeding cases.
2) Histopathological findings of all cases were chronic atrophic hyperplastic gastritis ; 42 cases of them were accompanied with intestinal metaplasia and 10 cases without metaplasia.
3) In the transitional zone the mucosal reconsruction such as gland atrophy, intestinal metaplasia and proliferation of lymph follicles was most evident. And erosions, scar of erosions and of ulcer were concentrated in this zone.
4) Mucosal reconstructive changes of bleeding cases were more intensive than nonbleeding cases. It is therefore thought that bleeding happens much more frequently in the stomach which shows sever reconstruction.
5) In one case atypical cell nest and in another case small adenocarcinoma were proved in scars of ulcer in the transitional zone of lesser curvature.
6) It may be thought that heterotopic proliferation of muscularis mucosae towards lamina propria which I proved is a important cause for bleeding and gastric distress.
7) The operative treatment of gastritis and gastritic bleeding seems enough to attain the purpose by partial gastrectomy including the transitional zone.
8) The influences of operative manipulations upon histological findings of gastric mucosa are proved to be negligible ones such as dilatation of capillary and slight edema of lamina propria.
