STUDIES OF THE SURGICAL TREATMENT FOR PRIMARY CHRONIC GASTRITIS

We studied indication for surgical treatment of the primary chronic gastritis from 160 cases of resected stomach and follow-up results of these cases.
1. To decide the indication of surgical treatment of primary chronic gastritis, we abvocated “5 Type Classification.” It was valuable for clinical and gastroscopical uses. (of. J.J.S.S. Vol. 55, No, 4, Page 361)
2. We found primary chronic gastritis in rather yanger adult than older ones, and its pathological changes were not co ordinate to the age of the patients.
3. Most of patients, in primary chronic gastritis, were complained epigastric pain, hematoemesis, bloody stool, and uncomfortable feeling on gastric region. Especially epigastric pain had relationship to atrophic changes of mucous membrane of the stomach.
4. Acidity of gastric juice was slightly influenced by pathologic changes of the gastric body.
5. There was no close relationship between duration of disease and its pathologic changes. But it might be said that long durated cases showed atrophic changes of mucous membrane.
6. Among primary chronic gastritis 22.5% of our cases showed marked stomach bleeding and it had tendency to repeat. In these cases microscopically these were Type I. (Serious inflammation or marked edematous swelling of the interstitial tissue) and macroscopically Edematous Swelling Type.
7. Gastroscopic examination was most suitable method to decide operation.
8. Rather small resection included the antrum was enough for treatment of primary chronic gastritis. Follow-up results were getting better gradually, but diffuse and productive cases were necesary for some duration to take satisfactory results.
9. Among primary chronic gastritis, operative indication was Fine Wrinklea Type and Atrophic Type, especially long durated or no effects on conservative treatment.
