CONDITION OF ADAPTATION AFTER ILEOCECAL RESECTION FROM THE VIEW POINT OF THE SHIFT OF SERUM PROTEIN FRACTION
For either recognition of the limit of operative indication or discussion of the operative-techniques, recognition of condition of adaptation after operation specially after resection is very important problem.
Condition of adaptation after resection of th ileocecal region which is a peculiar part among the intestines was studied from the view point of serum protein fraction.
Fifty cases excluding malignant tumor cases were studied. End to end anastomosis was used in all the cases. The results obtained were as follows:
According to the preoperative change of serum protein fraction, period of complete adaptation is much different. In cases in which no or slight change was observed, it became to normal 1-3 months after operation. In cases in which severe change was observed, each fraction became to normal and showed complete adaptation 6 months after operation. In those cases recovery of γ-globulin was most disturbed.
Essential factors disturbing postoperative adaptation were stenosis of anastomotic area and continuance of abnormal intestinal movement. In order to prevent those non-adaptating factors, the mesenterium must be handled as carefully as possible at the time of operation and oblique anastomosis at the ileum stump might be applied.
According to the resurts obtained by the author, ileocecal resection and end to end anastomosis gives the favorable postoperative condition of adaptation and it must be positively applied for ileocecal diseases.
