STUDIES OF CEREBRAL BLOOD FLOW AND METABOLISM
PART II. VARIATION OF CEREBRAL BLOOD FLOW AND METABOLISM AFTER LIGATURE OF THE A. CAROTIS EXT. (BOTH SIDES) FOR IDIOPATHIC EPILEPSY (46 CASES) AND CLINICAL RESULT OF OPERATION (136 CASES)

1st Surgical Department, Nagoya University School of Medicine (Director: Dr. Yoshio HASHIMOTO)

Takashi AMEMIYA

The variation of cerebral blood flow and metabolism after ligature of the A. Carotis ext. (both sides) for idiopathic epilepsy (46 cases) were as follows:
1) The values 1 week after operation (40 cases)
The average value of C.B.F. was 64.47 cc/100 gr. brain/min.; C.M.R.O2. was 3.60 cc/100 gr. brain/min.; C.M.R.gl. was 6.64 mg.gl/100 gr. brain/min. which were considerably increased than before operation. These values came near the normal values.
The average value of C.V.R. was 1.22 mmHg/cc. blood/100 gr. brain/min., which was considerably decreased than before operation. This value was slightly decreased than the normal values.
2) The values 4 weeks (32 cases), 1 month (17 cases) and 6 months (13 cases), after operation
These average values of C.B.F., C.M.R.O2. and C.M.R. gl. were slightly decreased than values 1 week after operation. But these values were considerably increased than the values before operation.
3) The values after neck operation (No Ligature of the A. carotis ext).
The values of C.B.F., C.M.R.O2. and C.M.R. gl after neck operation were considerably increased than the values before operation, but these increases are slightly than the values after ligature of the A. Carotis ext. The value of C.V.R. after neck operation was decreased than the value befor the operation, but this decrease is more slight than the value after ligature of the A. carotis ext.
But after 3 months, these variations came near the values before the operation.
4) Clinical result of operation are as follows:
-observation from 6 months to 5 years (136 cases)-
(1) 32 patients (23.6%) becam to have no epileptic convulsion at all.
(2) 62 patients (45.5%) became to have little epileptic convulsion.
(The aboves are favourable results.)
(3) 11 patients (8,4%) showed little changes.
(4) 31 patients (22.5%) showed no change.
(The aboves are no favourable result.)
