NON-PENETRATING TRAUMA OF THE ABDOMEN AND HEART, AND SO-CALLED NEUROGENIC ULCERATION OF GASTROINTESTINAL TRACT

First Surgical Department, Tokyo University School of Medicine (Director: Prof. Kentaro SHIMIZU)

1) Cases with non-penetrating trauma of the abdominal internal organs, including spleen, small intestine and pancreas, were represented. In such an injury associated with the automobile accident, the power of inertia might be emphasized to be one of mechanisms causing the nonpenetrating trauma.
2) Two cases of non-penetrating trauma of the heart (the contusion of the heart) were represented. They might be considered to be caused by the similar mechanism when receiving auto-accidents.
3) In some cases with non-penetrating trauma, the power of inertia could be an important factor of mechanisms producing such a trauma even in the abdomen and heart (just the same situation as the "contre-coup" injury in the head trauma)
4) Case with so-called neuroganic ulceration and perforation of the duodenum following the severe head-trauma was representened. The mechanism of the neurogenic ulceration of the gastro-intestinal tract was discussed by collecting the literatures.
5) Patients should be checked carefully for, at Ieast, several days even after trival accidents. When non-penetraling trauma of the abdomen, heart etc. will be suspected or diagnosed, proper treatment (including exploratory laparatomy) should be applied immediately.
