The enlarged splenic volume in Banti's syndrome was reduced by porta-caral anastomosis (Eck's fistula), but the pathological blood and bone marrow picture were not improved. In shistosomiasis japonicum chronica, the enlarged splenic volume was reduced by ligation of splenic artery, but the pathological blood and bone marrow picture were not improved. By splenectoniy these pathological blood pictures were almost improved.
It is evidently concluded that these pathological blood and bone marrow picture are based on the pathological spleen, and is suggested that the excessive destruction of blood corpuscle in congestive spleen is not a main cause for pathological blood picture.
The process of improvement in blood picture of these disease by splenectomy was compared with that of the other splenectomized patient such as gastric cancer, esophageal cancer, aplastic anemia, familial hemolytic jaundice, essential thrombocytopenic purpura and Felty's syndrome. Pancytopenia in Banti's syndrome, shistosomiasis japonicum chronica and Felty's syndrome was improved by splenectomy with similar process that was seen after removal of normal spleen. In familial hemolytic jaundice the anemia was improved immediately after splenectomy, but the platelet count was not changed. In essential thrombocytopenic purpura the erythrocyte count was not changed, but the platelet increased remarkably soon after splenectomy. In 2 cases of aplastic anemia, the blood picture was little changed by splenectomy.
