We studied 110 cases of resected stomaches which were diagnosed so-called spontaneous chronic gastritis, and obtained resultes as described below.
1) The macroscopic findings of mucous membrane of resected stomaches could be divided five groups which were;
(1) edematous swelling type, (2) rough nodulous upheave type, (3) flat small nodulous tipe, (4) fine wrinkled type, and (5) atrophic type.
2) Microscopicially it was also divided five different groupes becouse (1) serious inflamations of interstitial tissue, (2) remarkable multiplying of gastric glands, (3) so-called atrophic hyperplastic gastritis which means that propliatic gastric gland was becoming atrophic and deck epithel was increased (4) atrophic propliatic gland with inflamation of interstitial tissue, and (5) thin mucous membrane that was caused by serious atrophy.
3) In our cases type 1 and 4 were common comparing to the others. Macroscopically edematous mucous membrane showed marked infiltration of cells to interstitial tissue and edema. And at the same time atrophic granulated mucous membrane had tendency of atrophy of propliatic gastric gland.
4) The stomaches which were taken out from the patients who complained gastric symptomes for examples pain of chest and so on, were mostly type 4. Type 1 was found commonly from the patiens who had a history of emesis.
5) Chronic gastritis with gastric ptosis showed mostly atrophic picture on mucous membrane. And also propliatic muscle showed remarkable degeneration.
6) From the results of our studies we want to say that the groupes which had serious atrophy, gastroptosis, repeated emesis or bleeding and resist to medical treatment are indication of surgery.
7) The term of surgery which we mean is that partial gastric resection included a ntrumand body of the stomach. We believe that procedure is enough to the disease.
8) Serious cases of chronic gastritis required rather long time observation after surgery and we think it is good idea to give medical treatment to them. We believe that surgical procedure will be normalized the pH of stomach juice, too.
