STUDY ON HEPATIC CIRCULATION IN SURGERY OF PORTAL SYSTEM
Utilizing the hepatic vein catheterisation method, and estimating the hepatic blood flow by B.S.P. clearance method, the author made a study concerning the hepatic circulation, chiefly in the region of the portal surgery, and obtained the following results:
1) The estimated hepatic blood flow is inclined to decrease in cases of portal hypertension, especially in cases clinically diagnosed as hepatic cirrhosis. But no significant correlation is found between the estimated hepatic blood flow and the portal pressure or pathohistological cirrhotic changes in liver.
2) The hepatic arteriovenous oxygen difference is inclined to increase in cases of portal hypertension, especially in cases of hepatic cirrhosis.
3) There is no difierence between splanchnic oxygen consumptions of the group of portal hypertension and of the control group.
4) There are some cases of portal hypertension in which the hepatic venule pressure is not higher than usual. On the contrary the hepatic venule pressure shows always high in the cases having cirrhotic changes in liver.
5) The high portal pressure can be decreased most effectively by Eck's operation. This operation has a considerable influence on the hepatic circulation, including the fact that it decreases considerably the hepatic blood flow.
6) The hepatic (blood) circulation is less influenced by splenectomy than by Eck's operation. (author's abstract)
