CIRCULATORY DISTURBANCES IN SURGICAL OPERATION OF PULMONARY TUBERCULOSIS

Surgical Department, Tokyo University Branch Hospital

Tadayuki OOIDE

Circulatory disturbances induced by thoracoplasties or other operations for pulmonary tuberculosis were investigated in 126 cases.
The results were as follows
1) Blood loss most severely impaired the circulation, and if it exceeded 20 gm. per kg of body weight, patient was exposed to direct peril.
2) According to the tilt-test made immediately after operation, blood loss should be restricted within 10 gm. per kg.
3) Blood pressure of recumbent position was inadequate criteria for estimation of circulatory status, because the tilt-test often revealed hidden circulatory disturbances even when the recumbent pressure was normal.
4) Blood transfusion of slightly greater amount than the lost blood which was measured immediately by weighing method, added by inhalation of oxygen and fluid infusion would hold not only the recumbent pressure but also the change of the tilt-test in normal range. (author's abstract)
