TREATMENT OF HEPATOCELLULAR CARCINOMA WITH HEPATIC VENOUS AND/OR INFERIOR VENA CAVA INVASION

Hepatocellular carcinoma (HCC) with hepatic vein (HV)/inferior vena cava (IVC) invasion has a poor prognosis, but specialized descriptions are not recognized in the various national treatment guidelines and it is addressed under portal invasion, etc. In 2017, a paper was published reporting the efficacy of surgical resection for HCC with HV tumor thrombosis based on a nationwide survey of the Liver Cancer Study Group of Japan. Those results were included in CQ 16 of the Japan Society of Hepatology HCC Guidelines 2017. However, it is still difficult to treat patients with HV/IVC invasion. Molecular-targeted drugs, immune checkpoint inhibitors, etc. have recently been used, but their efficacy needs to be evaluated in the future. Based on recent knowledge, this article reviews the treatment of HCC with HV/IVC invasion.
