ADVANCED TRAUMA OPERATIVE MANAGEMENT (ATOM) COURSE
The Advanced Trauma Operative Management (ATOM) course is a one-day course combining didactic lectures with a live animal surgery laboratory providing trainees with the opportunity to practice the management of actual penetrating abdominal traumatic injuries and gain the skills needed for trauma surgery. This review covers several topics including the current activity of the Japanese ATOM, efficacy and controversial issues of the course, the new ATOM nursing course, and the relationship between military surgery and the ATOM course. Several reports have demonstrated the effectiveness of the ATOM course, which gives trainees self-confidence in performing trauma surgery. There are also some difficulties regarding the ATOM course, such as anatomical differences between humans and pigs, course logistics, and ethical issues. Recently, the ATOM nursing course was created and its effectiveness is being evaluated. After the start of the Iraq and Afghanistan wars, the importance of surgical management, especially damage control surgery, was elucidated, and military surgeons benefit from training in a live animal laboratory, such as the ATOM course. Finally, due to the number of mass shootings in the USA, the Hartford Consensus was established, which defined the role of police, fire departments, emergency medical services, and hospitals in response to mass shootings.
