A 58 year old man was admitted to evaluate his swallowing disturbance. Preoperative aortogram demonstrated right aortic arch with isolation of left subclavian artery. Operative findings revealed the absent blood flow through the proximal portion of left subclavian artery which connected to the left pulmoanry artery. Abnormal connection was divided and Kommerell’s diverticulum was mobilized from the esophagus. His clinical symptom was slightly reduced postoperatively.
