EXPERIENCE WITH SURGICAL MANAGEMENT OF BACTERIAL ENDOCARDITIS
This paper is purposed to present and discuss results of surgical treatment of Host Valve Endocarditis (HVE, 13 cases) and Prosthetic Valve Endocarditis (PVE, 5 cases) which we experienced in Department of Thoracic Surgery in Juntendo University Hospital between 1975 and 1981. 13 cases of HVE were divided into the three subgroups depending upon activity of infection at the time of operation.
1) Active phase of endocarditis: 6 cases were treated surgically, and one died from postoperative LOS.
2) Subacute or healing phase of endocarditis : 4 cases survived from surgery without a death.
3) Healed phase of endocarditis : 3 cases were treated surgically without operative death.
Overall operative mortality of surgically treated HVE was 7.7%, and no major clinical problem was experienced.
Regarding 5 PVE cases, two cases were treated surgically and only one case survived. Three patients treated medically and two of them died, one case survived and is doing well with long period chemotherapy. Prevention and treatment of PVE is still the major problem to be solved at the present time.
In conclusion, aggressive surgery is recommended to HVE due to low mortality rate.
