PREOPERATIVE HEMATOPORPHYRIN DERIVATIVE AND LASER PHOTORADIATION THERAPY TO REDUCE THE EXTENT OF SURGERY IN LUNG CANCER
Hematoporphyrin Derivative [HpD] has an affinity for malignant tissue and cytocidal effects are obtained by light stimulation of HpD in tumor. It is therefore possible to treat malignant tumor selectively by the application of HpD and laser photoradiation. An argon dye laser [Spectra-Physics Co. Model 171-08 and 375-03, 630nm wavelength] was used as a light source for the HpD excitation.
HpD and laser photoradiation therapy was performed preoperatively in 12 cases of bronchial tumors with invasion, and the effectiveness of this therapy to reduce the extent of resection necessary and to extend the surgical indications of surgery were evaluated. Successful results were obtained in 9 cases. Photoradiation therapy improved the condition of inoperable cases to the point where surgery could be performed in 4 out of 5 cases and the extent of resection was reduced from pneumonectomy to lobectomy in 5 out of 7 cases. Tumor cells were not seen in the resected bronchial stump except in a single case of adenoid cystic carcinoma and no tumor recurrence has therefore been observed in 8 out of 9 operable cases at 1 to 16 months. Photoradiation therapy can be indicated in cases of superficial invasion of tumor in the bronchial mucosa in onder to reduce the extent of surgery or to make inoperable cases operable.
