EFFECT OF REFLUX OF PANCREATIC JUICE INTO BILIARY TRACT ON OBSTRUCTIVE JAUNDICE

The purpose of this experimental study was to evaluate an influence of reflux of pancreatic juice into the biliary tract on obstructive jaundice. Male Wister rats were used and divided into the following 3 Groups. The extrahepatic bile duct was ligated and cut at just above the duodenum in Group I, near the hilum of the liver in Group III, and doubly ligated and cut at these two points with or without bile injection into the doubly ligated bile duct in Group IIB or Group IIA.
The following results were obtained : 1) Mean survival was 7.4 weeks in Group III, 5 weeks in Group II, but in Group I all died within 30 days with mean survival of 1.4 weeks. 2) Body weight decreased gradually in Groups I and II, but increased naturally in Group III. 3) The histological changes of the liver and bile duct were most predominant in Group I, with significant proliferation of the connective tissue and developed into biliary cirrhosis within 4 weeks, although it occurred after 6 weeks in Group III. 4) Pancreatic enzymes, especially activated phospholipase A2, were observed in bile and intrahepatic bile duct in Group I.
These findings revealed that a refluxed pancreatic juice caused severe damage in hepato-biliary system with obstructive jaundice, and strongly suggests that it constitutes one of factors that were prolonged the obstructive jaundice.
