GROSS PATHOLOGY OF CARCINOMA OF THE ESOPHAGUS IN RELATION TO ITS MALIGNANCY
The gross pathology of squamous cell carcinoma of the esophagus was studied on 196 specimens. The classification in relation to its malignancy was made as follows.
1) Protuberant type of low malignancy, 2) Protuberant type (highly malignant), 3) Regularly ulcerative type, 4) Irregularly ulcerative type with high margin, 5) Irregularly ulcerative type with low margin, 6) Superficial type, and 7) Exophytic type. In the less malignant protuberant type, biopsy specimen shows well differentiated squamous cell carcinoma and in the highly malignant protuberant type, it shows poorly differentiated squamous cell carcinoma.
The well diffeneiated protuberant type and regularly ulcerative type are low malignant while protuberant type (poorly differentiated) and irregularly ulcerative type with low margin are highly malignant. Other types show inter mediate malignancy. The malignancy related to gross pathology was studied. For tumors of the regularly ulcerative type, resection is preferable even when it is only palliative and the resection should be combined with pre- and postoperative irradiation. For highly malignant types such as the poorly differentiated protuberant type and the irregularly ulcerative type with low margin of far advanced stage, indication for resection should be carefully considered particularly if the tumors are at an advanced stage.
