IMPERFORATE ANUS: THE PAST, PRESENT AND FUTURE
I. History
History of imperforate anus in Europe and the United States has been summarized elegantly by F.D. Stephens in 1971. The main point he made was that puborectalis muscle has an important relationship with the post-operative anal function.
In Japan, the name "Sako" a Japanese term for imperforate anus, was coined by S. Hanaoka in 1830. He treated the disease by cutting the perineum and dilating the opening by bougie.
2. Classification
In 1970, Stephens and Smith have proposed the international classification. In the presentation, the problems of the classification were discussed.
3. Operative management and follow-up investigations
Based on the expedience of our own, the rational of the choice of various procedures for the particular type was discussed. For high type anormaly, we prefer to use an abdominoperineal approach (Nichidai method). As to the post operative function of the high type, we observed that the cases with incontinence decreased, although the patients with constipation increased.
Finally, factors responsible for the development of constipation were discussed by analyging the data obtained by X-ray and manometric studies.
