RADICAL OPERATION OF TETRALOGY OF FALLOT BY USING SURFACE INDUCED DEEP HYPOTHERMIA
Open-heart surgery for Tetralogy of Fallot by using surface induced deep hypothermia was presented. The method of anesthesia has been described already. As a rule, however, we follow the method advocated by Dr. Okamura. Cardiac resuscitation of our method was made by injection of resuscitating agent through Teflon catheter which was stiched into the aortic root instead of manual massage.
The case of Tetralogy of Fallot which was presented in our film was 5 years old male. Radical operation was made for 50 minutes of circulatory arrest at 19.5 °C of the lowest body temperature. The second case was double-outlet right ventricle (origin of both great arteries from right ventricle) with pulmonic stenosis. The patient was 8 years old male. Radical operation was made for 60 minutes circulatory arrest at 18.5 °C of the lowest body temperature. These cases had good cardiac resuscitation and excellent results in follow-up study.
Open-heart operation under profound hypothermia introduced by surface cooling is a safe and effective method for Tetralogy of Fallot, because of completely bloodless and motionless operative field. And we are able to make at ease to repair ventricular septal defect and pulmonic stenosis by using this method.
